What is IHP?
Integrated Health Partners (IHP) is an FQHC-controlled clinically integrated network, dedicated to advancing value-based care and payment reform for traditionally underserved populations in San Diego and Riverside counties. With 342 thousand attributed lives and a network spanning 97 clinical sites, IHP is at the forefront of actively reshaping health care within the state of California. By leveraging best practices, fostering critical conversations, and driving change through data, IHP is committed to meeting the evolving needs of patients and improving health outcomes.
