IHP's Quality Initiatives
IHP Clinical Committee
IHP’s Clinical Committee is composed of clinical leaders from each of our member health centers. The charge of the committee is to set strategic priorities for the network related to the clinical quality and value of care for patients. The committee has six overarching goals:
- Establish a focus on quality/value measures for the network.
- Achieve oversight of health plan programs and interventions.
- Achieve oversight of workgroups reporting to the clinical committee.
- Advise on network coding strategy.
- Advise on network behavioral health strategy.
- Advise on the network’s glide path to health equity.
The committee meets monthly and is led by its chair, Dr. Marie Russell (TrueCare), and vice chair, Dr. Sarah Fatland (Vista Community Clinic).
Quality Process Improvement
The IHP Quality Program is a data-driven approach to assist health centers in improving their overall quality of care while helping to lower total costs and improve patient access to services. This approach allows the IHP Quality Program to target areas for improvement and implement strategies to help enhance the care of our populations.
Our quality team collaborates with payors to identify key performance metrics, ensuring that data is accurate and care gaps are closed. To help address quality needs, we have designated patient navigators who assist Health Centers in engaging patients throughout their care processes.
IHP’s partnership with payors also works to develop projects aimed at improving performance in pay-for-performance contracts. Our Quality Liaisons are Subject Matter Experts in HEDIS measures and are always available to assist any health centers in need of additional resources.
Care Management
IHP’s Care Management Program helps clinicians to identify patients who require extra attention and services. This program is designed to ensure that patients receive the right care, at the right time, in the right place. IHP Care Management utilizes data to segment patients based on stratified clinical risk. This approach allows for the optimization of care teams and leveraging of resources to improve the overall efficiency of organizations.
The IHP Care Management Program can help healthcare providers improve their value-based contract performance by providing high-quality patient care that improves outcomes, and positions member health centers to earn incentives for meeting or exceeding performance targets.
