Community Health Systems, Inc. is a non-profit 501 (c) (3) 330 HRSA Grantee with Federally Qualified Health Center (FQHC) status. Established from the roots of Bloomington Community Health Center in Bloomington, CHSI has grown with community health centers in the counties of Riverside, San Bernardino, and San Diego. These centers have been developed in accordance with standards established for safety net providers by the U.S. Department of Health and Human Services (HHS), the Health Resources Services Administration (HRSA), the Public Health Service (PHS), and the Bureau of Primary Health Care (BPHC). The mission of CHSI is not to compete with other clinics that provide healthcare services, but to serve the indigent population and meet their healthcare needs.
Imperial Beach Community Clinic is a private, non-profit healthcare organization that operates two clinics: Imperial Beach Health Center at 949 Palm Avenue in Imperial Beach, California, and Nestor Community Health Center at 1016 Outer Road in San Diego. For more than 39 years, Imperial Beach Community Clinic has provided primary healthcare to people of all ages in Imperial Beach and surrounding communities of southwest San Diego County.
Neighborhood Healthcare Founded in 1969 and incorporated in 1971, Neighborhood Healthcare is an award-winning organization that is known for excelling at its mission. The mission has remained the same over the years: to provide quality health care and promote wellness to everyone in our communities, focusing on those most in need. As a private, non-profit 501(c)(3) community health organization, we serve as a safety net for the community by providing 238,003 medical, dental and behavioral health visits to 67,222 people annually. Neighborhood Healthcare began as an all-volunteer neighborhood clinic in Escondido and now has almost a dozen health centers located throughout San Diego and Riverside counties.

Since 1979, the San Diego American Indian Health Center has provided high quality, compassionate primary, dental, and behavioral health care to all people in our community regardless of their ability to pay. San Diego Family Care is a private, non-profit community health center corporation, nationally recognized for its high quality of care, affordability and cultural competency. Established in 1972, our network of clinics in Linda Vista and City Heights provide a wide variety of services to a diverse range of families living in San Diego County. Our clinics are privileged to deliver primary care medical, mental health, dental services, and clinical trials to 30,000 patients annually. San Ysidro Health improves the health and well-being of our community’s traditionally underserved and culturally diverse people. SYH provides compassionate and innovative care to over 90,000 patients through a vast and integrated network of program sites including: medical clinics, dental clinics, behavioral health centers, HIV centers, WIC nutrition centers, mobile medical units, school-based health centers, chiropractic care, Pediatric Developmental Clinic, Teen Clinic, Senior Health Center and Program of All Inclusive Care for the Elderly (PACE) Center. Originally established along the border in 1969, today SYH provides high-quality, compassionate care at sites located throughout the South Bay (National City, Chula Vista and San Ysidro), Southeast San Diego, and El Cajon. SYH’s unique services include the San Diego Children’s Dental Center at Rady Children’s Hospital offering oral care for medically fragile children, the Adult Special Needs Dental Clinic housed within SYH’s King Chavez Health Center, Urgent Care services located at Chula Vista Medical Plaza, and Mi Clinica – SYH’s retail clinic at Northgate Gonzalez Market. Daily, 1,200 dedicated staff members provide culturally proficient and vital health services to make lives better for the most vulnerable families within San Diego County’s impoverished communities.
St. Vincent de Paul Village/ The Village Health Center Provides nearly 25,000 visits per year, including more than 3,000 dental visits, the federally qualified St. Vincent de Paul Village Family Health Center offers primary and pre-natal care, along with well child exams, mental health counseling, addiction and dental services. Specialists also provide access to dermatology, optometry, ophthalmology, cardiology, gynecology and chiropractic care. We are especially grateful to partner with the UCSD Combined Residency program, whose residents dually train in both family medicine and psychiatry. The participation of several other schools, including SDSU, USD, Bethel Seminary, Argosy University is also critical to the success of the Health Center.
TrueCare began in 1971 as North County Health Services (NCHS), a grassroots effort to serve Migrant Farmworkers in the rural areas and farming communities of North San Diego County. Today it operates multiple clinic sites in five cities offering medical, dental, behavioral health and OBGyn services to its 62,000 patients. TrueCare also operates an extensive WIC food supplement program for its patients and the community serving over 18,000 clients. TrueCare is NCQA certified with PCMH level III certification. TrueCare is a high performing FQHC surpassing quality and fiscal performance metric set by HRSA for community health centers. In 1973 with the aid of UCSD, TrueCare (then NCHS) became a Community/Migrant Health Center and incorporated as a private, non-profit 501(c)(3). The mission and vision of TrueCare is as relevant today as it was at its inception “to improve the health status of our diverse communities by providing quality healthcare that is comprehensive, affordable, and culturally sensitive.” This is consistent with the ACA triple aim and we are committed and dedicated to achieve these aims.
Vista Community Clinic Welcoming. Trustworthy. Innovative. Caring. These qualities have defined Vista Community Clinic since it first opened in the basement of a local animal shelter in 1972. Vista Community Clinic quickly became the health care safety net for the area’s poor and uninsured by giving them access to the high quality health services that they needed and deserved. Today Vista Community Clinic is recognized as a key regional health provider with five state-of-the-art clinics treating almost 60,000 patients each year.
