Integrated Health Partners recognizes the power of their Community Clinic Health Center (CCHC) members. Borne out of the communities they serve and with an enduring appreciation of ethnic and cultural diversity, they are all non-profit organizations dedicated to delivering primary care and preventive health services at the highest possible standards. This unique collaboration of so many like-minded organizations gives Integrated Health Partners its strength.
By working together, the CCHC members ensure and enhance vital community services and population health objectives for communities across Southern California. As such, Integrated Health Partners seeks primary care capitation contracts primarily for Medi-Cal, Medicare and Covered California under a direct agreement with health plan payors.
This direct payor relationship allows for clear alignment of health plan and CCHC objectives for the joint patients they serve. In addition to the standard Healthcare Effectiveness Data and Information Set (HEDIS) measures, Integrated Health Partners’ CCHCs are all committed to advancing quality through the Patient-Centered Medical Home accreditation process as developed by NCQA. With many of the CCHC’s sites at Level 3 certification and on-line with San Diego Health Connect, San Diego’s Health Information Exchange, Integrated Health Partners is capable of cutting-edge quality incentive programs not often available in the safety net provider community.
